You need a picture. I got mine from here.
First duplicate the base and set it to hard light.
Now make a new layer and fill it with #00A2FF. Set it to overlay at 100% opacity.
Go to Layer >> New Adjustment Layer >> Hue/Saturation and set your settings to mine:
Master:
Saturation: +15
Lightness: +15
Blues:
Hue: -8
Saturation: -14
Cyans:
Saturation: -11
Duplicate the base layer again and drag it to the top and set it to hard light at 31%.
Go to Filter >> Blur >> Blur More. Do it one more time.
Your layers should look like this:
Layers >> Merge Visible.
I also added a border and some text.
You're all done!
