You need a picture. I got mine from here.
First duplicate the base and set it to screen.
Now click on Layer >> Adjustment Layer >> Hue/Saturation. Match your settings to mine:

Make a new layer and fill it with #000CAE. Set it to lighten at 100% opacity.
Your layers should look like this:
Layers >> Merge All
I also added a 5px white border and 1px #B4B4B4 border.
You're all done!
